    Your membership allows you to visit the site as often as you need during your loved one’s recovery journey. This site is set up in sections according to areas of care. As a member, you can pull information from any section at any time, and have the choice to focus only on what you need at the time you need it—because we all know that too much information at once can be overwhelming.

    Care Partners Resources Main Areas for learning and finding resources:

    Acute or Life Threatening: Acute care is the critical care phase, generally requiring 24-hour care. The goal of acute care is to stabilize patients so they can be sent home or to another facility.

    Non-Acute Hospitalized/Not Life Threatening: Your loved one will be coming home soon. Life or death decisions are no longer a matter of concern; it is now time to plan for the future.

    Long-Term Home/Rehab/Long-Term Care Facility: Your loved one has entered a maintenance phase of care. This phase varies dramatically from one person to another. It can involve being back at home or in a facility. It can involve rehab at a facility or having a therapist come to the home. The common factor is that your loved one has medical problems under control.
