Long Term Care
Home/Rehab/Long-Term Care Facility
Your loved one has entered a maintenace phase of care. This phase varies dramatically from one person to another. It can involve being back at home or in a facility. It can involve rehab at a facility or having a therapist come to the home. The common factor is that your loved one has medical problems under control.
Long-term care does not mean your loved one’s health challenges have all been cured. Nor does it mean that the current status will stay the same. Your loved one could improve, or perhaps could improve in one area but not in another, or could improve in one area and even worsen in others. Long-term care can mean years of the same, or it can mean continuous change. Monitoring health changes is just the beginning. Equally important is understanding how health changes can change your eligibility for new care services.
