- Do you feel a discharge to home is feasible? definition of discharge: point at which patient leaves hospital and either returns home or is transferred to another facility
- If not, what does insurance/Medicare/Medicaid pay towards a facility?
- Which facilities are covered by insurance/Medicare/Medicaid/?
- Is it possible for us to schedule a visit to some of the care facilities in the area?
- Is the hospital or rehabilitation center willing to allow me or a selected person to observe the staff while they perform the daily care for our loved one?
- When will it be okay to bring in some items from home?
- Does the hospital have someone who can come to the home to identify the things that will need to be adapted to make my home safe and user friendly for everyone, loved one and care partner? If so, how do I set that up?
- Is there financial assistance for items needed to adapt the home?
- What is the qualified status identification for insurance/Medicare/Medicaid to determine the amount of coverage for caregiving and other assistance?
